Note: All volunteers are required to go through a background check.

Please fill out this Volunteer Application and return to a Northwest Little League Board Member.

Medical Release Form Information

Please bring signed medical release forms to your first practice. You should have received the form in an e-mail after registration, but if you did not, follow these steps.

1. On our web site, click "Member Login"
2. Sign in with the account you created during registration. If you forgot your account information, you can have it sent to you.
3. Click on Past Registrations
4. Click on Print Form next to your child and it will bring up the medical release form.
